Obtaining A Copy of Your MRI Medical Records
Because of medical privacy laws, we are unable to release records by phone, fax, or online.
MRI reports from Southern Oregon Open MRI can be obtained from:
Health Information Management DepartmentWhen you come to the above address, you will be asked to provide a photo ID and then sign an authorization in order for us to give you a copy of your records. Or, at your request, we can send the actual CD to your physician.
